REPUBLIC ACT NO. 9225
CITIZENSHIP RETENTION AND ACQUISITION ACT 2003
- The applicant must have been a natural born Filipino Citizen
- The loss of Philippine citizenship by the applicant must have been on account of naturalization as a citizen of a foreign country.
- Only applicants residing in Guam, Commonwealth of the Northern Mariana Islands (CNMI), Palau, Federal State of Micronesia (FSM), Republic of Marshall Island (RMI) are accepted. For other states, territories and countries please see nearest Philippine Embassy or Philippine Consulate in its jurisdiction.
- The applicant must personally submit documentary requirements to the Philippine Consulate General, Agana Guam at the time of the appointment.
- The applicant will be given the date and time of the oath of allegiance before a Philippine Consular Officer.
- Payment of Fee: US$ 50.00
NOTE: PLEASE PRESENT THE ORIGINAL DOCUMENTS AND SUBMIT ONE (1) XEROX COPY OF EACH OF THE REQUIRED DOCUMENTS.
IMPORTANT: APPLICATION FORM SHOULD BE COMPLETELY FILLED-OUT, ANSWERS TYPED OR PRINTED LEGIBLY WITH BLACK OR BLUE INK, AND ALL REQUIREMENTS SUBMITTED MUST BE COMPLETE, OTHERWISE APPLICATION SHALL BE RETURNED UNPROCESSED.
Documentary Requirements
- Duly accomplished Application for Retention/Reacquisition of Philippine citizenship
- PSA-issued Birth Certificate
- Two (2) 2’’ x 2’’ recent colored photos with white background showing the applicant’s full facial front view.
- Original Certificate of Naturalization/ Certificate of Citizenship (derivative of parent) or other evidence of naturalization
- Original valid foreign passport
- For married women, PSA-issued Marriage Certificate or issued by the Office of Vital Statistics
- Present previous Philippine Passport (optional)
Other documents that may be required to confirm personal identity and birth information of applicants, where the name currently used by applicant is different from the name on the Philippine birth certificate, due to the following reasons:
- Due to legal proceeding:
- Authenticated copy or proof or evidence of authority to change name or Court or INS/BCIS order;
- Affidavit of Discrepancy regarding change of name; and
- Two public (2) or private documents showing the correct name of applicant
- For married women who choose to adopt the surname of her husband and for divorced persons or have had their marriage annulled but are still using marriage surname:
- Original or certified true copy (CTC) of the marriage certificate
- For adopted children:
- Authenticated copy and photocopy of the dispositive portion of the Court Order on Adoption; and
- Authenticated PSA birth certificates before and after adoption
- For legitimate children due to subsequent marriage of parents
- Annotated PSA-issued Birth Certificate; and
- PSA Marriage Certificate of parents
Derivative Citizenship for Unmarried Minor Children of Applicants
If the applicant wishes to have his/her unmarried minor children acquire Philippine citizenship at the same time, the following documents of the child must be submitted:
- Original Birth Certificate
- Foreign passport
- Two (2) 2’’ x 2’’ recent colored photos with white background showing the applicant’s full facial front view
- Fee: US $ 25.00
NOTE: The Consular Officer reserves the right to require additional documents from the applicant, to prove his/her identity and/ or citizenship, and ensure accurate and complete personal data entries, pursuant to the New Philippine Passport law (R.A. 11983) and the Foreign Service Act (R.A. 7157).

Report of Marriage between Filipinos abroad or a between a Filipino and a Foreigner Abroad
The marriage of a Filipino abroad should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy or Consulate having jurisdiction over the locality where the event took place. In Guam, the Commonwealth of Northern Marianas Islands (CNMI), the Republic of Palau, the Federated States of Micronesia and the Republic of the Marshall Islands, the Report of Birth should be filed at the Philippine Consulate General in Agana, Guam.
Basic Requirements for Report of Marriage
- Duly-accomplished and signed Report of Marriage (ROM) form (4 copies)
- Certificate of Marriage (present original together with 4 copies) issued by the Office of Vital Records or any other appropriate of the state, territory or country where the marriage took place.
- Copies of valid passport of both the husband and the wife (present original passports together with 4 copies for each passport)
- PSA Birth Certificate of Filipino spouse (present original together with 4 copies)
- PSA Certificate of No Marriage (CENOMAR). (original and 4 copies)
- Processing Fee of US$25.00
Additional Requirements
IF REPORT OF MARRIAGE (ROM) IS BEING FILED MORE THAN ONE (1) YEAR AFTER THE MARRIAGE
- Affidavit of Delayed Registration of Marriage explaining the reasons for delayed registration
- Additional Fee of US$25.00 for Affidavit of Delayed Registration
IF FILIPINO SPOUSE WAS PREVIOUSLY MARRIED AND DIVORCED, OR PREVIOUSLY MARRIED AND MARRIAGE WAS SUBSEQUENTLY ANNULLED
- One original or certified true copy and four (4) additional copies of the Philippine Judicial Decree of Divorce or Annulment or annotated PSA Marriage Certificate reflecting the annulment.
IF FILIPINO SPOUSE IS A WIDOW/WIDOWER
- One original or certified true copy and four (4) additional copies of the Death Certificate of deceased spouse

Important Note: Nationals of a number of countries with which the Philippines maintains diplomatic relations, including the United States, may avail themselves of VISA-FREE entry into the Philippines, provided their stay will not exceed 30 days. Please contact the Philippine Embassy/Consulate General for information on which countries are extended this privilege.
US citizens who wish to stay more than 30 days in the Philippines and nationals of countries who are not extended the 30-day visa free privilege should apply for a visa before traveling to the Philippines.
Returning Balikbayans or former Filipinos may also enter the Philippines without a visa and stay for a period of one (1) year, provided that they have a passport valid for 6 months beyond the intended stay in the Philippines, a roundtrip ticket, and proof of former Philippine citizenship (e.g. old Philippine passport or NSO-authenticated birth certificate). Foreign spouses and their minor children may avail themselves of this privilege if traveling with the Balikbayan.
General Requirements for a 9(a) Temporary Visitor Visa
- Duly accomplished visa application form;
- Original and photocopy of applicant’s passport with six (6) months validity beyond intended stay in the Philippines;
- Proof of financial capacity (any of the following):
- Bank statement of the latest six (6) months;
b. Employment certificate with remuneration;
c. Proof of business income; - Letter of guarantee from the sponsor with proof of financial capacity
- Bank statement of the latest six (6) months;
- Documents proving ties to the home country/country of residence (any of the following)
a. Employment certificate;
b. Property documents - Round-trip flight itinerary to country of origin or next country of destination;
- Hotel accommodation or proof of lodging
- Itinerary
- For minors not travelling with a parent/guarding (additional requirement like letter of permission from a parent/guardian
- Visa Fee: US$30.00 (Single-entry, valid for 3 months for applicants from the US and other visa-free countries) / US$40.00 (Single-entry, valid for 3 months for applicants from non-visa free countries)

Report of Death of a Filipino Citizen Abroad
The death of a Filipino citizen abroad should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy or Consulate having jurisdiction over the locality where the event took place. In Guam, the Commonwealth of Northern Marianas Islands (CNMI), the Republic of Palau, the Federated States of Micronesia and the Republic of the Marshall Islands, the Report of Death should be filed at the Philippine Consulate General in Agana, Guam.
Basic Requirements for Report of Death
- Duly-accomplished and signed Report of Death (ROD) form (4 copies)
- Information on the deceased, form is attached (4 copies)
- Certificate of Death (present original together with 4 copies) issued by the Department of Health or Office of Vital Records or any other appropriate of the state, territory or country where the death took place.
- Copies of itinerary to include full flight details and mode of transport (original plus 4 copies)
- Four (4) copies of valid passport (or identification card) of deceased
- Processing Fee of US$25.00
FOR CREMATED REMAINS WHICH WILL BE BROUGHT TO THE PHILIPPINES (AND WILL BE ISSUED A CONSULAR MORTUARY CERTIFICATE)
- Certificate of Cremation issued by the Crematory (original plus 4 copies)
- Notarized Mortuary Certificate
- Burial-Transit Permit (Urn)
- Passport or identification card (4 copies) of the person who will travel with the urn
- Fee of US$25.00 for Consular Mortuary Certificate
- Fee of US$ 25.00 for Certificate of No Objection
FOR HUMAN REMAINS WHICH WILL BE BROUGHT TO THE PHILIPPINES (AND WILL BE ISSUED A CONSULAR MORTUARY CERTIFICATE)
- Notarized certificate (original plus 4 copies) issued by the Mortuary Director that the body was properly embalmed or cremated, and that the casket contains only the body of the deceased.
- Certificate of No Contagious Disease issued by health authorities of the state, territory or country
- Burial transit permit (original plus 4 copies)
- Name, address and contact number of the receiving funeral parlor in the Philippines
- Fee of US$25.00 for Consular Mortuary Certificate
- Fee of US$ 25.00 for Certificate of No Objection

Report of Birth of Child Born Abroad of Filipino Parent/s
The birth of a Filipino should be reported to and registered with the Philippine Statistics Authority (formerly NSO) through the Philippine Consulate General in Agana, Guam having jurisdiction over the locality where the event took place (i.e. Guam, Commonwealth of Northern Mariana Islands, Republic of Palau, Federated States of Micronesia & Republic of Marshall Islands).
REQUIREMENTS:
- Report of Birth application form, typed or printed legibly in black or blue ink. Forms should be notarized if submitting by mail. – 4 originally signed accomplished form.
- Certificate of Live Birth – 4 photocopies & bring original
- Philippine Statistics Authority (PSA) of formerly National Statistics office (NSO) Marriage Certificate or Report of Marriage of the parents - 4 photocopies & bring original
- PSA/NSO Birth Certificate of father & mother to verify Middle Name of the Child 4 photocopies & bring original
- Current/ Valid Passports of both parents or government-issued identification card (if parent do not have a current/ valid passport) - 4 photocopies & bring original
- Passports of parents (valid at the time of the birth of the child)- 4 photocopies & bring original
- Valid U.S. Permanent Resident Card (Green Card) or a copy of the U.S. Visa of Filipino parent/s - 4 photocopies & bring original
- If applicable, U.S. Naturalization Certificate of parent(s)- 4 photocopies & bring original
- If applicable, PSA/NSO Death Certificate of parent(s)- 4 photocopies & bring original.
- If applicable, dual citizenship documents of parents & child – 4 photocopies & bring original.
- Negative Certification of Birth Record from PSA (CRS Form 1) for applicants whose age is five (5) years old and above.
If Parents are Not Married:
- Affidavit to Use the Surname of Father (AUSF), executed by the mother or the child (with Sworn Attestation of the mother, if child is less than 7 years old) - 4 photocopies & bring original
- Affidavit of Admission of Paternity (AAP) executed by the father or Declaration of Paternity issued by the Vital Statistics Office or Private Handwritten Instrument– 4 photocopies & bring original
- PSA/NSO Certificate of No Marriage of mother - 4 photocopies & bring original
If Parents are Married after Child’s Birth:
- Joint Affidavit of Legitimation by subsequent marriage of parents - 4 photocopies & bring original
- PSA/NSO Marriage Advisory - 4 photocopies & bring original
- Affidavit of Delayed Registration of Birth explaining the reasons for delayed registration, if Report of Birth is filed more than a year after the child is born - 4 photocopies & bring original
- Processing fee: Report of Birth is $25.00, Affidavit of Delayed Registration of Birth is $25.00, AUSF is $25.00, AAP is $25.00 & Certificate of Registration is $25.00 Affidavit of Legitimation is $25.00 (non-refundable), payable in cash, or postal money order, bank draft, certified check or cashier’s check, made payable to “Philippine Consulate General". Personal checks and credit cards are not accepted.
Those filing through the Office of Consular Affairs, all affidavits, including Report of Birth form, are to be notarized in the Philippines.
Note: The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted.
The Consular Officer may require additional proof or documents from applicant to determine the child’s citizenship, identity, or eligibility for registration of birth under Philippine laws.
